plan / periodic reassessment
- TIARA
tracks the patient assessment and all recommended classes
plus class attendance and reassessment. It also provides the
capability of documenting patient life experiences and
preferences as required in the Standard.

educational plan, evaluation, follow up, and outcomes measures
- TIARA
provides the mechanism to track the assessment, educational
plan and completion, patient goals and the achievement of
those goals through follow-up assessment.

provides the data required for an effective CQI program. The
reports can assist the program coordinator in identifying
program weaknesses so that a systematic approach to
improvement can be achieved.
